I hereby make a gift of $500 to purchase an engraved memorial brick to be installed on the UCI campus. I understand that UCI determines the style, design and location of the brick. 

I hereby make a gift of $10,000 to purchase a memorial bench, with plaque affixed, to be installed on the UCI campus. I understand that UCI determines the style and design of the bench and plaque and that the location of the bench will be determined by UCI in consultation with our family. I understand that funds may be raised to purchase the bench and that, if we elect this option, I will collect checks and forward them together to the UCI Foundation.
